On-line Questionnaire

Postby Steve Thurston » Tue Jan 27, 2004 1:26 pm

Hi all!

With the help of Jeff Cameron I now have a more accessible trauma questionnaire.

It is now possible to complete and submit the form via the site and should take no longer than 10 min.

http://www.14thcentury.org/impact.asp

This form will only be on-line until the end of Feb and the more people who complete the form the more accurate the interpetation can be.

Please be assured that the data collected will only be used to help outline the potential of WMA in the past and will not degregate the WMA community in any way.

Impact trauma means any hit taken that hurt.

Stress or strain refers to anything from torn ligaments to aching joints at the end of a session and that was not the result of receiveing a hit.

I am must grateful to any who complete this questionnaire, to those who have already taken the time to complete the old questionnaire thank you and your data has already been entered.

Thanks to all those who have helped and in advance to those who complete the forms

Steve Thurston
