This is a great place to start -- the forum and web page that is. The web page itself has so much information that it might seem overwhelming but after a few days of browsing that'll soon pass and you'll find that it answers your questions. Since I'm new as well, I'll try to answer your questions as best I can.
1-Wear comfortable athletic gear -- to start anyway. I know some people wear period-authentic shoes, but I personally can't make or afford that sort of thing yet. I'm sure some people around here can help you better in this category.
2-This web page has quite a lot of information about basic sword, as well as shield, halberd, dagger, and other period weapons. Actually joining ARMA will open up access to much more information. There may be a study group in your area as well <img src="/forum/images/icons/smile.gif" alt="" /> . Finally, there are a bundle of texts here, and at your local bookstore, that may help, including Medieval Swordsmanship by John Clements.
3-Depending where you live you can get foam at a local hardware store or marina (or many other places). I personally don't build my own foam weapons since I've had bad results.
4-This is a good place to start. Personally, I'd recommend practicing stances and basic cuts first.
Hope I've helped, but others will soon follow with much better advice.
